An experienced Band 7 Community Diabetes Specialist Nurse will join the Community Diabetes Service in Harrow at Honeypot Lane Health Centre. The role works within a multidisciplinary team to deliver high‑quality, evidence‑based diabetes care across community settings, including autonomous practice and close collaboration with primary, secondary, and community services.
Responsibilities include specialist clinical assessment, service development, structured education delivery, and transformation of community diabetes services in Harrow.
Main duties of the job
* Support the Team Leader in delivering a high‑quality specialist Community Diabetes Nursing Service and contributing to service planning and delivery.
* Provide expert clinical assessment, management, and review of patients with diabetes, ensuring safe, evidence‑based, patient‑centred care.
* Work collaboratively as part of the multidisciplinary team, liaising with primary, secondary, and community care services to coordinate care pathways.
* Lead and support the development, implementation, and evaluation of specialist nursing interventions and service improvements.
* Use project management and clinical leadership skills to contribute to service development aligned with national guidance, local priorities, and best practice.
* Maintain accurate, timely, and comprehensive clinical documentation and records in line with professional and organisational standards.
* Contribute to the development and review of clinical guidelines, policies, and procedures, ensuring high standards of care within the service.

Person specification
Education/Qualification
* Degree in Nursing or equivalent.
* Current registration with the NMC as a registered General Nurse.
* Relevant post‑registration training in diabetes (e.g., master’s or diploma in diabetes management).
* Independent Nurse Prescriber, or willingness to undertake.
* Supervisory skills training.
* Mentorship training.
Experience
* Minimum of 3 years’ experience managing complex diabetes cases.
* Experience managing a clinical caseload.
* Experience training students and supervising others.
* Experience organising and implementing training/teaching programmes for healthcare professionals and clients.
* Excellent group education and presentation skills.
* Experience conducting audits and/or research.
* Experience working in a multi‑disciplinary team and in community settings.
* Experience applying behavior change techniques.
Skills & Knowledge
* Leadership ability in a clinical speciality.
* Advanced assessment and treatment skills for clients with diabetes.
* Strong team‐working and multidisciplinary collaboration skills.
* In-depth knowledge of evidence‑based practice in diabetes care.
* Ability to interpret health and social policies and guidelines and translate into practice.
* Problem‑solving, prioritisation and clinical reasoning based on experience and knowledge.
* Basic IT skills – Microsoft Word, PowerPoint, Excel.
