Are you a nurse or allied health professional with an MSc in Advanced Clinical Practice or have completed an accredited portfolio route to Advanced Clinical Practice? The Blandford and Shaftesbury Adult Community Mental Health Teams are seeking an Advanced Clinical Practitioner.
As an Advanced Clinical Practitioner, you will be dynamic and highly motivated, with exceptional assessment, risk management, and care formulation skills. You should have experience working with adults with significant mental illness in a multidisciplinary setting. Strong communication skills, clinical decision-making abilities, and understanding of MHA, MCA, and safeguarding legislation are essential. You will also prescribe, offer senior clinical support, and triage referrals.
This is a full-time position, working Monday to Friday, 9:00 – 17:00. A job share for 18.75 hours will be considered. We welcome discussions about flexible working. There are posts based with Blandford CMHT and Shaftesbury CMHT in North Dorset, collaborating with other clinical and operational leads across our CMHTs.
Driving is essential for this role.

Your responsibilities include undertaking assessments, nurse reviews, managing a small group of patients, supporting carers, and signposting for ongoing support. You will have a job plan to work across the four pillars of advanced practice, collaborating with community teams, social care, and other agencies to benefit patients. You will lead initiatives to monitor and improve standards through supervision, evidence-based practice, clinical audits, research, and education.
You will plan and manage complete episodes of care, conduct independent assessments, and make complex clinical decisions regarding pharmacological considerations and clinical outcomes. Additionally, you will contribute to the planning and development of integrated services.

Employment requires an Enhanced DBS check, which the Trust will cover. Applicants enrolled in the DBS update service can present a valid DBS certificate instead of a new check.
