We have positions available for experienced health care assistants who are keen to join our responsive health care team and share their experience with the staff and patients they are supporting. This is a great opportunity for those wanting to join our bespoke team within DCHS.

Are you a compassionate and experienced Healthcare Assistant with a keen interest in patients with enhanced care needs? As a Responsive Healthcare Assistant, you will play a crucial role in supporting patients with enhanced care needs as they transition into our service. You will work closely with the Multi-disciplinary team to ensure that patients receive the highest standard of care. An element of Speciality training will be provided to equip you with the skills needed for this role.

As part of this role, you will be required to be flexible and to change location at short notice. Therefore, a driving licence and access to a vehicle will be essential. Main duties will include the day-to-day delivery of care guided and planned by Registered Nurses and other Registered Professionals such as Therapists and Advanced Clinical Practitioners:

1. Assisting patients with daily activities, including personal hygiene, mobility, and feeding.
2. Providing emotional support and reassurance to patients and their families.
3. Monitoring and recording patients' baseline observations and reporting any changes to the nursing staff.
4. Supporting the nursing team in delivering care plans tailored to each patient's needs.
5. Helping patients settle into the ward environment and ensuring their comfort and safety.
6. Participating in speciality training to enhance your skills and knowledge.
7. Maintaining accurate and up-to-date patient records.
8. Collaborating with other healthcare professionals to provide holistic care.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes, and increasingly, in people's own homes and via virtual consultations.
