Job Description
Care Navigator
Able to work under pressure
EMIS clinical system
Answer the telephone dealing with or redirecting enquiries.
Handle general enquiries referring to appropriate person where necessary
Make new, follow-up and triage appointments
Processing online requests from patients (eConsult)
Completing work flow tasks
Responding to emails
Assisting with QOF (Quality and Outcome Framework) training will be given
Assisting with Key Performance indicators training will be given

Personal Specification
· Excellent IT skills
· Good General office procedures
· Ability to prioritise and manage a workload
· Well organised with good time management
· Work well both in a team and independently
· Ideally knowledge of EMIS – full training will be provided.
