Key Responsibilities
* Ensure compliance with all office systems and procedures.
* Ensure rota is allocated, maintained and assigned to appropriate staff.
* Updating of care plans and providing support to the Care Teams.
* Conduct care staff supervisions and assist the other Care office staff with aspects of their role.
* On call duties.
* Develop strong relationships with the customers, their families, and other healthcare professionals.
* Maintain accurate and up-to-date records of care provided and any changes in customer’s needs.
* Good communication and listening skills.
* Problem solving ability and a drive to succeed.
Requirements
It’s important to us that all Caremark Care Coordinators have the following qualities:
* Experience of domiciliary care, care co-ordination, and a working knowledge of the care industry.
* Previous experience with Access Care Planning and People Planner desired.
* U.K. Driving Licence.
* Excellent communication skills and the ability to build rapport with staff, clients and their families.
* NVQ level 2 or higher in Health & Social Care
