FULL TIME - 37 hrs
Isle of Wight Homeless Pathway provides support to adults who are homeless or at risk of homelessness across various properties, including a dedicated women's only service. Our aim is to provide person-centred support, empowering clients to gain life skills necessary for independent living. We collaborate with various agencies to maximize support and opportunities for clients.

JOB PURPOSE
To lead client case management and support clients in the community to ensure their safety and help them achieve their goals. To provide housing and specialist support, making clients feel valued and secure, and preparing them for the next steps.
MAIN RESPONSIBILITIES
1. Assess clients' needs and develop individual support plans to meet their goals.
2. Produce and review comprehensive risk assessments for clients.
3. Manage a caseload of clients on a one-to-one basis.
4. Support clients in maintaining accommodation and achieving outcomes.
5. Engage with local authorities, health providers, and other agencies to maximize opportunities for clients.
6. Lead client activities to develop practical life skills.
7. Assist clients with finances, rent payments, arrears, and accessing benefits.
8. Signpost clients to community resources, education, training, employment, or volunteering.
9. Support clients using trauma-informed and psychologically informed approaches.
10. Adhere to safeguarding policies and procedures.
11. Maintain health and safety standards for clients, visitors, and premises.
12. Provide basic first aid until help arrives (training provided).
13. Work confidently and efficiently when lone working.
14. Maintain accurate records on the client management system in line with GDPR.
15. Ensure confidentiality and professionalism in representing the organization.
16. Perform any other duties in the interest of the organization.
ROLE REQUIREMENTS
* Enhanced DBS check with Barred List(s) is required upon appointment.
* Full driver's license and access to a vehicle.
* Ability to work flexibly across multiple sites.
* Willingness to work nights and weekends.
* Ability to work alone and communicate effectively both verbally and in writing.
* Proficiency in Microsoft Outlook, Excel, and Word.
* Confident, assertive, and self-motivated.
* Effective team player and able to respond calmly in crises.
* Genuine concern for homelessness and related issues.
DESIRABLE CRITERIA
* Experience managing a caseload of clients.
* Understanding of risk assessment and outcomes-based support.
* Knowledge of support needs of homeless or complex clients.
* Familiarity with voluntary and statutory agencies, housing regulations, and benefits system.
