We have an exciting opportunity for a hard‑working and enthusiastic professional to join our Intermediate Care Team in Blantyre Life.
In this role, you will be working with people discharged from a hospital setting who require a period of reablement, recovery and assessment to determine any needs. This may involve home visits to support discharge.
Responsibilities
* Gather, select and analyse information to support assessment decisions.
* Identify the underlying cause of any functional difficulty.
* Provide equipment and/or adaptations as direct solutions, or refer to other statutory or voluntary services.
* Work autonomously and within tight timescales, demonstrating responsibility for your own learning needs.
Qualifications
* HNC in Occupational Therapy Support or willingness to work towards achieving this qualification.
* Relevant experience in intermediate care or similar settings (preferred).
Working Hours and Salary
* Hours: 21 hours per week (permanent).
* Salary: £21,855 – £22,479.
