Overview
A Band 7 Specialist Nurse, Community Practitioner is invited to join the Adult Community Learning Disability team. The team applies MDT principles with a friendly specialist approach to supporting individuals with a learning disability to maintain independence within the community, addressing health inequalities and promoting wellbeing.
Main duties
The Specialist Nurse will:
* Provide a high level of nursing care and treatment to Service Users with high care needs due to complex difficulties and/or associated risks.
* Deliver high-quality nursing care, assessment and therapeutic intervention to adults with complex difficulties as part of a multidisciplinary team.
* Manage complex risks competently in line with Trust and statutory guidance.
* Facilitate access to mainstream primary care services for adults with mental health and/or learning disabilities and cognitive problems; advise and support agencies on meeting duty of care.
* Identify health inequalities and advise on reasonable adjustments to improve service accessibility.
* Support team managers with team development and practitioner performance; contribute to non-medical prescribing where applicable.
* Participate in planning, development and evaluation of clinical services, leading defined projects with the Clinical Manager, Clinical Lead and Team Manager.

* First Level Nursing Qualification and Registration
* Student Nurse Assessor Safeguarding Children and Adults Training
* Degree or equivalent qualification or experience in evidence-based psychological therapies relevant to Pathway
