Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Mental Health Team to work full time based in Tameside.
The salary for this permanent Social Worker job is up to £45,091 per annum.
Responsibilities
* To act as a Social Worker for people with mental health problems.
* Assessment, formulation and monitoring of risk.
* To monitor, review and evaluate care for the service user in collaboration with the multi-disciplinary team, referring agent, family/carer and service user, where appropriate.
* To liaise with all agencies and members of the multi-disciplinary team, adult social care & MCFT to ensure the needs of the service user are met.
* Establish and maintain communication with individuals and groups on the caseload about difficult or complex matters, in such a way as to overcome any problems in communication.
* To offer peer supervision to local authority Community Support Worker/Social Workers as appropriate.
* To maintain accurate, up to date records on the local authorities electronic record system, for all service users referred a receiving assessment, care treatment and support in the mental health team.
* To participate in the Local Authorities Mental Health Duty system on a rota-basis and be able to offer a rapid response to service users immediate needs, including those arising from crises and support our health colleagues with understanding and implementing service users social care needs.
* To promote the physical health & wellbeing of service users.
* To undertake duties in ensuring Safeguarding is everyone’s business in relation to an adult with care and support needs who may be at risk of abuse or neglect and any action required.
* To participate in the AMHP rota if appropriate, or train to be an AMHP.
* To support service young users that are transitioning from children’s services, acknowledging the complexities involved with this work, including EHCPs, SEND and multi-agency working.
* Be accountable for your own practice and ongoing learning, contribute to the learning of the team through supporting colleagues and student placements.
* Comply with departmental policies & procedures & quality standards.
* Exercise decision-making with reference to legal and ethical literacy.
* To engage in regular in regular supervision and Annual Development Reviews.
* To develop the service & assist in continuous improvement programmes.
* To undertake the duties in accordance with the hours as may be required by the service.
Requirements
* A social work qualification or equivalent.
* Understanding of relevant legislation.
* Registered or eligible for registration.
