Overview
Health Care Assistant role with Coventry and Warwickshire Partnership NHS Trust. The closing date for applications is 16 September 2025.
Responsibilities
The post will be part of a team that provides short-term support to individuals referred to PCDU. The team will work with people who require immediate short-term support to engage with community support networks. Support may relate to employment, housing, financial issues, interpersonal relationships or other social issues. The primary focus is short-term work using ordinary resources within the community and urgent care.
* To assist clinicians in the assessment of health and social need of individuals referred to PCDU.
* Provide emotional support to patients, relatives and carers.
* Under the direction of clinical staff, assist individuals to access community support to enable them to leave the PCDU and use appropriate community services to meet their needs.
* To assist and engage in practical activities with individuals to enable them to live as independently as possible and promote social inclusion. This will involve working with individuals in short-term practical activities, including engaging with housing providers, General Practitioners, social care, other secondary care mental health services, voluntary organisations and benefits/welfare agencies.

Qualifications
* NVQ Level 2 or Equivalent
Experience
* Proven experience of working within a mental health care setting
