This is an exciting opportunity for an enthusiastic and motivated nurse with relevant experience to join our established integrated intermediate care team. The Collaborative Care Teams have undergone developments and in the CQC report, our team is described as an outstanding example of community working.

The service receives referrals from a wide range of health and social care professionals, with the aim of supporting patients with a health related crisis at home. The goal is to avoid an acute hospital admission and/or support early discharge. We also provide support to patients who require out of hours district nursing and provide input to people accessing intermediate care beds in the community.

In addition, the Collaborative Care Teams provide support to patients within their own homes that require IV therapy, this includes clinical assessment of patients, management and escalation of unwell patients, patient centred care planning, administration of IV medication, peripheral cannulation and on-going care of cannulas, venepuncture and management of Central Venous Access Devices.

This is a 24 hour a day 7 days a week service operating 365 days a year therefore the post holder will be expected to work shift patterns to cover the service including Early, Lates, Long Days, Nights or a combination of both and have access to own transport.

We are a national centre for telemedicine and introduced telehealth to the UK offender healthcare sector and also provide the service to patients in their care homes. Community matters to us and we are supported by 400 dedicated volunteers and have strong links with Bradford University, Craven College and Leeds City College to ensure we inspire the workforce of the future.

KEY ROLE - MANAGE CARE

Work in collaboration with the Team Leader, and other members of the team to assess individuals’ health and social care needs,

Provide time limited programmes of care for individuals:
to prevent unnecessary admission to hospital or

to facilitate early discharge from hospital or

to meet rehabilitation needs to promote and maintain independence

Contribute to the planning, implementation, monitoring and review of programmes of care for individuals with the Team Leader, and other members of the team and ensure a smooth transition of care to other providers e.g. District Nurse, Home Care

Manage and Co-ordinate caseload in absence of Team Leader or Sister.

Assess risk, which affects the health and safety of individuals, and care givers.

Ensure agreed standards of care are achieved

To support individuals in self-management of long term conditions

Promote people’s equality, diversity and rights.

Enable people to manage disability, loss and change.

Contribute to the protection of individuals from abuse.

Develop innovative approaches to engage traditionally hard to reach and challenging groups

Work in collaboration with other agencies to develop pathways of care for individuals to facilitate early discharge from hospital or prevent unnecessary admissions to hospital.
