The post holder will have responsibility to provide a single point of access for all referrals into Adult Mental Health services.

To review all referrals to identify any gaps in the referral process, with a view to strengthening existing joint partnership systems to improve early and appropriate referrals.

To collate and analyze statistical data from the current database to facilitate the process.

To triage all referral information, making autonomous clinical judgments in order to prioritize and allocate the referral to the appropriate professional within our service or suggest alternatives to the referrer. This is complete in line with the Mental Health Triage Scale, risk management process, the Mental Health Measure (Wales) 2010, and the UK Mental Health Triage Scale Guidelines (2015).

To triage phone calls from external agencies and the general public, providing brief interventions during the calls.

To conduct holistic assessments and formulate risk assessments.

Main duties of the job

An exciting opportunity has arisen within the Adult Mental Health Service. The role involves being part of a developing and expanding team to improve access and response to mental health services.

The post is for a dedicated Single Point of Access Professional, open to qualified mental health professionals.

The role includes triaging all referral information, prioritizing, and allocating to appropriate professionals/services, as well as conducting face-to-face assessments.

The team is new and developing within the health board, encouraging development and support. The ethos is to deliver high-quality care and support to individuals experiencing mental health concerns.

Detailed job description and main responsibilities

Person specification

Qualifications

* RMN registration, occupational therapy, or social work qualification
* Possession of a postgraduate qualification (diploma, degree, or higher) or evidence of working towards one

Experience

* Significant post-qualification experience as a Band 5/6 or equivalent in acute adult/community mental health
* Experience in teaching and training health and/or social care professionals and designing educational inputs (e.g., conference presentations, publications)
