Main duties of the job
The role involves completing robust and holistic health care assessments, leading multidisciplinary team meetings, care planning safe and effective packages of care, and supporting individuals with complex care needs to maintain their quality of life. This role regularly involves providing support and guidance to both our clients, their families, and external professionals, in order to achieve the best outcomes. The team is responsible for upholding the guidance provided by the National Framework for NHS Continuing Health and NHS‑funded Nursing Care (Revised 2022) and therefore, comprehensive training is provided to ensure this specialist knowledge is gained.

Job responsibilities
We are looking for a registered professional who is experienced in working with people who have complex physical and mental health needs.
NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs.
The Complex Cases Team is responsible for the decision making and commissioning processes, working closely with the Local Authorities throughout Cambridgeshire and Peterborough.
Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a home‑based role, and post holders will be expected to be able to travel across Central East.
You will be joining a fast‑paced team that employs the use of holistic nursing, inquisitive thinking and a focus on high quality. The nature of the role is autonomous and challenging but with the benefit of supportive colleagues.

Job Purpose
The Continuing Healthcare Specialist Assessor will provide advice with respect to NHS Continuing Healthcare, NHS‑funded Nursing Care and joint funded care, to clients, health and social care professionals and other agencies, within relevant work streams.
The Job Description is a generic role and covers all work streams within NHS Continuing Healthcare.
The Continuing Healthcare Specialist Practitioner will lead and coordinate the NHS Continuing Healthcare assessment process. This will include carrying out nursing assessments and multidisciplinary team meetings, ensuring the care domains within the National Framework for NHS Continuing Healthcare are applied against each assessment and a detailed rationale given for each decision made. The post holder will make recommendations based on clinical evidence if clients have a Primary Health Need and therefore are eligible for NHS Continuing Healthcare.
The post holder will be responsible for liaison with clients and/or their representatives, health and social care professionals, and other agencies with respect to sourcing suitable placements and/or packages of care which are clinically effective and cost effective.
Person Specification
Experience
* Experience of carrying out complex nursing/healthcare assessments for clients with health and social care needs.
* Experience of care planning for clients with complex health and social care needs.
* Experience of working in partnership between health and social care and wider system partners.
* Experience of Vulnerable adults legislation.
* Experience within NHS Continuing Healthcare / NHS‑funded Nursing Care.
* Experience of assembling specialist packages of care which are tailored to the needs of individual clients.
Qualifications
* Valid professional registration as a Registered Nurse (General / Adult / Mental health / Learning Disability) OR Registered Allied Health Professional (OT / Physiotherapist / Speech and Language Therapist).
* Educated to degree level or appropriate level equivalent experience.
* Continuing Care Specialist Training or Qualification.
