This has created additional commissioned hours that cannot be covered with current staffing levels.
We are actively recruiting so this will be until we are able to recruit to the permanent roles Job description The Community Living Service provides 24 hour support to adults with disabilities living in small, shared homes within the community.
These are individuals’ own homes, and Support Workers play a key role in ensuring each person is supported to live safely, comfortably and with dignity while maintaining choice, independence and control within their shared environment.
Support Workers assist with all aspects of daily living, including personal care, communication, mobility, meal preparation, household tasks and accessing community activities.
They work in accordance with individual care plans, behaviour support plans, health action plans and risk assessments to promote wellbeing, inclusion and positive outcomes.
The role involves contributing to a person centred, safe and supportive environment, building professional relationships with the people we support, and working closely with families, colleagues and external professionals.
Support Workers are responsible for accurate record keeping and must uphold the values and professional standards required by Social Care Wales.
This is a 24/7 service, and staff are required to work a rota that includes days, evenings, weekends, nights and sleep in shifts.
This role will be based in Chirk in Wrexham.
