What you will do: Undertake Care Act assessments and reviews for adults with mental health needs. Review and update care and support plans to promote recovery, independence, and wellbeing. Monitor and manage risk, ensuring proportionate safeguarding responses. Apply relevant legislation including the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005. Contribute to statutory mental health processes and reviews. Work collaboratively with Community Mental Health Teams, psychiatrists, nurses, and allied professionals. Attend multi-disciplinary reviews, professionals' meetings, and case discussions. Identify and respond to adult safeguarding concerns in line with local procedures. Complete risk assessments and contribute to protection planning. Maintain accurate, timely, and high-quality case records. Work in line with Social Work England standards and council policies. Participate in supervision, training, and continuous professional development. Qualifications: Degree in Social Work (BA / MA / DipSW). Registration: Registration with Social Work England (SWE) is mandatory. Experience: Post-qualifying experience within adult social care or mental health services.
