Overview
You will establish links with community service providers and gain up to date knowledge of the services they provide to reduce digital exclusion.
You will support patients who are digitally excluded to access training and resources and help them develop the skills and confidence to access virtual care and electronic systems.
You will be proactive in identifying opportunities to reduce digital exclusion, improve the uptake of digital care and self-management of health and wellbeing.
Youll work in close collaboration with colleagues across the Trust, in the community and local authority partners. This is primarily a patient-facing role and you will have direct contact with inpatients (people staying in hospital) and outpatients (patients accessing treatment at appointments).
