Discharge Co-Ordinator - Manchester Royal Infirmary
An excellent opportunity has become available for an enthusiastic and highly motivated Discharge Co-Ordinator to join our supportive Integrated Discharge Team at Manchester Royal Infirmary.
We are looking for an individual who can be part of our fast-moving, innovative, and ambitious team; dedicated to supporting our patients and working with clinicians and ward staff to manage the patient’s journey from admission to discharge.
The ideal candidate will be dynamic, adaptable, and able to identify issues, provide solutions, and demonstrate a team-focused attitude with patient care and safety at the forefront of decision-making.
Key Responsibilities:
1. Identify medically fit patients.
2. Report delayed discharges in line with Reason to Reside guidelines.
3. Communicate effectively with departments to ensure safe and timely discharges.
4. Assess patient’s care needs on discharge.
5. Work flexibly across various settings.
6. Be competent with IT systems.
7. Establish partnerships with wider service providers and agencies.
8. Coordinate with hospital discharge services for equipment needs.
9. Communicate effectively with patients, families, and services.
10. Assist patients and carers in accessing information or signposting.
11. Understand social care legislation including the Mental Health Act, Mental Capacity Act, and Care Act.
