We have an exciting opportunity to recruit a highly motivated Health Care Assistant to join our busy Community Heart Failure service at Liverpool University Hospital NHS Foundation Trust.
This is a permanent position within a well-established team of specialist nurses. We are looking for an experienced Health Care Assistant with skills in phlebotomy and recording patients' clinical observations.
The role involves providing patient education, supporting symptom management, preventing unnecessary hospital admissions/readmissions, and maximizing patients' quality of life. Clinical support will be provided by Heart failure specialist nurses.
The service operates across various settings including community clinics, patients' homes, and in collaboration with primary and secondary care. Candidates need to be mobile and able to visit patients across Liverpool in a timely manner.
This role requires flexibility and the ability to work within an evolving, proactive, and multidisciplinary team.
