Job summary
The Community Staff Nurse willwork Out of Hours as part of the Urgent Community Response team within theCommunity hub. The community staff nurse will need to be able to prioritiseurgent patient care in the community setting dependent on the patients needsand clinical condition. The post holder will be required to deliver clinicalassessments and care interventions to ensure that the individualised needs ofthe patients are maintained.
Main duties of the job
The community staff nurseworking in the Out of Hours team must be able to triage patient calls utilisingthe appropriate triage template on system one, communicating with the patientto ensure that the appropriate care intervention is provided to patients in atimely manner. The community staff nurse will need to be able toprioritise patient care dependent on the patients needs and clinical condition. The community staff nurse will provideindividualised patient care plans ensuring high standards of clinical practiceare maintained through continuing personal/professional development.Will ensure high standards of clinical practiceare maintained through continual professional development, research audit andclinical supervision. To support and deliver care to patients in thecommunity setting who are on a Urgent Community Response caseload or have aurgent community response condition or illness.. The community staff nurse role is to improveclinical outcomes for patients by providing clinical interventions within thecommunity setting during Out of Hours. This will reduce prevent inappropriatehospital admission. The community staff nurse will work closely allteam members of the Community Urgent Community hub and members of themulti-disciplinary team. Work in partnership with members of the Health andSocial Care Teams in the delivery of services to achieve collaborative andintegrated working.
