We are recruiting a full‑time Band 5 Brent Urgent Care Practitioners to join our Brent Unplanned Care service, based at Sudbury and Wembley.
This is an exciting opportunity to be part of an innovative community service delivering acute, hospital‑level care directly in patients' homes. The Brent Urgent Community response service provides short‑term, intensive care for adults with acute conditions who would otherwise require admission to hospital.
Working within a multidisciplinary team of specially trained registered professionals, including GPs, you will support earlier discharge, prevent avoidable hospital admissions, and ensure patients receive the right care, in the right place, at the right time.
You will have access to home‑based monitoring equipment, point‑of‑care testing, and the ability to deliver a range of clinical interventions, including intravenous therapy, enabling you to practise autonomously while being fully supported by an experienced team.

Main duties of the job
The post holder will work both autonomously and collaboratively with Acute Trusts, the Adult Community Nursing Team, and a range of health and care partners, including statutory and voluntary agencies, to deliver high‑quality, person‑centred patient care.
* Work on a rotational shift basis within the Harrow Hospital at Home service, operating between 8am and 8pm, seven days a week.
* Effectively prioritise and manage own workload, working with a high level of autonomy and minimal supervision.

Person Specification
Education/Qualifications
* BSc degree or equivalent in Physiotherapy
* Evidence of continuous professional and personal development relevant to rotations
* Registration with the Health Professions Council
* Wide ranging student experience including musculoskeletal outpatients, adult/older people's rehabilitation, community rehabilitation and neurology placements.
* Valid HCPC Registration.
* Registration with a special interest group relevant to the rotations
Experience
* Able to manage a caseload efficiently
* Demonstrate professional independence, while recognising boundaries of own practice
* Effective Physiotherapy skills to be able to assess, goal set, treat and manage rehabilitation patients particularly within a multi‑disciplinary team.
* Problem solving approach to clinical interventions
* Able to keep logical and accurate patient records
* Research project carried out at undergraduate level
* Able to present information, written and orally, in a clear and logical fashion
* Effective interpersonal skills to liaise with all professionals to enhance patient care pathways
* Good team working skills
* Knowledge of NHS structure
* Presentation skills to lead and teach in‑service training sessions
Skills and Knowledge
* An understanding of evidence‑based practice pertaining to Physiotherapy
