Are you looking for a change? If so, your community Integrated Neighbourhood Team needs you! We are looking for a Senior Paramedic to join our dynamic multidisciplinary team supporting patients in their own hours once discharged from hospital or to prevent unnecessary hospital admissions.
You will provide holistic, safe, patient‑centred, effective and evidence‑based clinical assessments and interventions in the community. In turn, we will support your learning and development with a robust competency framework and training opportunities.
You will be part of an integrated multi‑disciplinary team (MDT), which includes Physiotherapists, Occupational therapists, Nurses, Community Specialist Practitioners, Band 4 Assistant Practitioners and a expansive unregistered workforce.
This service is 8am‑8pm 7 days a week therefore staff work variable shifts and some weekends.
Main duties of the job
* Complete holistic assessments of service users in their own homes, escalating for clinical support when necessary.
* Develop and deliver person‑centred care plans.
* Support with the co‑ordination hub within the locality.
* Line‑manage junior staff and contribute to their development through supervision, 1:1s and training.
* Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and deliver a competent clinical service.
* Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
* Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures.
* Coordinate shifts and delegate/allocate care to the team in accordance with policy.
* Act as a point of escalation for staff to report to on discovering any issues or deteriorations.
* Undertake further training as required in order to develop extended skills and enhance patient care.

* BSc or equivalent experience in Paramedic Science

* Recent relevant experience within the community setting including management of the frail complex patient.
* Working with a range of professionals in Health or Social Care.
