Overview
Dorset Healthcare are looking for Community Therapy Assistant Practitioner to join our supportive Purbeck ICRT based here in Wareham. The multi-disciplinary team work with adult patients in their own homes or care homes to promote independence and improve quality of life by supporting them to remain at home. Therefore being a car driver is essential for this role. You will be joining a motivated, caring, inclusive and compassionate team. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence. The role of Therapy Assistant Practitioner may also be required to work with patients on the ward at Swanage Hospital to support their rehab goals and help them to return home. We offer opportunities for further learning and development, including the chance to apply for Nursing, Physiotherapy and Occupational Therapy Apprenticeships.
Responsibilities
* Carry out holistic assessments, formulate treatment plans and monitor progress against set goals, under the indirect supervision of a registered therapist.
* Carry a clinical caseload of patients with orthopaedic health conditions including falls and trauma, ensuring high quality, evidence based interventions are delivered and recorded according to professional standards and legal requirements.
* Work autonomously in the community to visit patients while adhering to local and Trust guidance on lone working.
* Contribute to training and supervision of rehabilitation assistants and participate in regular in-service training and supervision.
* Support patients with rehabilitation goals and help them regain independence, potentially including ward-based work at Swanage Hospital as needed.
Requirements / Qualifications
* Full-time post of 37.5 hours per week.
* Hours 08:00-20:00, 7 days per week with weekend enhanced pay.
* Car driver is essential for this role.
* Must have the ability to carry a clinical caseload and deliver interventions that are patient-centred and evidence-based.
* Willingness to work across community settings and local hospital wards as required.
* Current Skilled worker sponsorship to work in the UK is not eligible to apply (if applicable to candidate).
