The post holder will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.

The Manager will develop and coordinate a multi-professional approachproviding evidence-based care to all those who use the service.This collaboration will harness an integrative agency approach so that physical, psychological and social care agencies are working together in providing holistic care to individuals who require its support.The post holder will be responsible for the continued development of the CLDT and will ensure service delivery is of a high professional standard, in line with Health Board policies and procedures.


Main duties of the job

To work within the policies, procedures and guidelines set out by the Health Board and professional bodies. Operate within the guidelines set by the NSF, Clinical Governance, Risk Management and NICE guidelines.

Establish and maintain effective care through the holistic and comprehensive assessment of service user's needs, planning, implementation, and review in accordance with PTHB and relevant Learning Disability policy and procedures.

Person Specification


Qualifications

* Registered Nurse Learning Disabilities
* Postgraduate qualification or significant relevant experience in related field
* Evidence of personal and clinical development
* Extensive knowledge of a wide range of learning disability health issues
* Knowledge and application of the Mental Health Measure Knowledge of research and evidence-based interventions both new and historic. Ability to underpin these with practice related experience Knowledge of current legislation Knowledge of the Improvement Cymru Action Plan for Wales
* Experience of specialist research in learning disabilities
* Educational experience/Teaching qualification
* In-depth medication management skills, and ability to demonstrate how these are used


Experience

* Experience of managing teams
* Community, hospital and acute experience
* Experience of providing supervision in group and individually
* Ability to provide presentations of a significant depth to large audiences, with confidence


Other

* Enthusiastic & self-motivating
* Ability to make own transport arrangements for home visits / to travel between sites
* Welsh speaker/learner
* Ability to lead a team confidently, professionally and sensitively
* Good care planning and problem-solving skills
* Clinical and management leadership skills
* Ability to work to tight deadlines
* Ability to balance conflicting issues with sensitivity, thought and understanding
* Positive attitude towards those with severe and enduring mental health issues
* Ability to act on own initiative, but able to balance the need to ask for advice
* Ability to lead/manage a team compassionately and professionally

£46,840 to £53,602 a yearper annum pro rata
