Job Summary
Wootton Vale Healthy Living Centre is looking for an enthusiastic and skilled practice nurse to join our friendly and forward‑thinking primary care team. This position can be offered on a full or part‑time basis with flexible options available.

As a key member of our clinical team, you will deliver a wide range of nursing services and support patients in managing chronic health conditions. We are particularly interested in someone who can take a proactive, holistic approach to patient care.
Main Duties of the Job
* Management and review of patients with long‑term conditions (e.g., diabetes, asthma, COPD, hypertension)
* Supporting long‑term condition clinics and QOF‑related work
* Providing high‑quality general practice nursing services such as immunisations, wound care, cervical screening, health checks and lifestyle advice
* Working autonomously while contributing to the wider multidisciplinary team
* Supporting practice initiatives to improve patient outcomes and access

We are a friendly surgery with a list size of over 18,000 patients, across two sites: Wootton Vale (MK43 9JJ) and Shortstown Surgery (MK42 0GS). We pride ourselves on good appointment access and a strong teamwork ethos, which includes an established clinical pharmacy team, GPs, nurses, physician associates, clinical liaison and admin teams. We are also a tier‑2 sponsoring practice and will support applicants requiring a tier‑2 visa.

Person Specification (Experience)
* Essential: Skilled in long-term condition management with confidence running LTC clinics
* Essential: Knowledgeable in relevant clinical guidelines and best practice
* Essential: A great communicator with a compassionate, patient‑centred approach
* Essential: Organised, proactive and able to work both independently and within a team
* Desirable: Competence in managing and prescribing for LTCs
* Desirable: Experience using GP clinical systems such as EMIS/SystmOne (if applicable)
Qualifications
* Essential: A relevant nursing degree.
* Essential: Current NMC registration with an active PIN.
* Desirable: Independent Nurse Prescriber (V300) qualification.
