About the role
An exciting opportunity has arisen to join the Specialist Palliative Care Team. We are looking for an experienced and highly motivated nurse who is passionate about palliative and end end of life care. Working as an autonomous practitioner you will support patients with complex palliative symptoms or support needs, ensuring they have timely access to expert symptom control, psychological support, information and advise.
The role covers a seven day service working across hospital and community.
Main duties
* Act as a role model and clinical expert within Specialist Palliative Care, both within the Trust and externally.
* Facilitate clinical care by providing expert assessment. Determine priorities of care and advise on complex symptom control, emotional, psychological, spiritual and social care.
* Contribute towards a multi-professional approach in the management of patients within Specialist Palliative Care, especially in relation to ongoing care needs.
* Work collaboratively with primary care colleagues through the implementation of and attendance at Gold Standards Framework meetings, other associated forums .and shared governance initiatives.
* Work in partnership with other health care and Specialist Palliative Care providers such as care homes and hospices, referring appropriately and facilitating a seamless service for patients.
* Establish and maintain effective communication with patients and carers/relatives.
* Demonstrate knowledge in relation to pattern of disease, markers of disease progression and understand the range of treatment available.
* Demonstrate knowledge of drugs used for the control of complex symptoms.
* Develop own ‘expert’ clinical knowledge and participate in the development of others' clinical knowledge.
* Undertake clinical supervision of nursing colleagues on an individual or group basis.
* Provide appropriate support and advice to bereaved carers known to the Specialist Palliative Care service and to refer on to local bereavement services as appropriate.
* Act as a core team member of the Specialist Palliative Care MDT.
* Recognise ethical and legal issues which have implications for nursing practice and take appropriate actions.
* Promote the use of nationally recognised end of life care tools to improve the quality of care.
* Continually monitor standards of care and participate in the improvement of care through benchmarking, clinical audit and research.
* Evaluate and disseminate research findings and facilitate evidence-based practice
* Working within role of clinical nurse specialist support Lead Nurse where required in the running of services at Rosewood Centre
Key requirements
* Registered Nurse
* First level ( Honours) degree or willing to undertake
* Evidence of continued professional development
* Recognised teaching qualification
* Post registration experience of Band 5 or above in a relevant area
* Multi-professional working
* Working autonomously
* Evidence of working with people with life limiting complex health needs
* Audit/ research experience
