In your day to day you will be expected to do the following:
* Conducting assessments to identify the needs of older adults.
* Coordinating care plans and support packages to promote independence.
* Facilitating hospital discharges and ensuring effective transitions to community care.
* Collaborating with healthcare professionals, families, and community services.
* Advocating for service users to ensure their voices are heard and needs are met.
* Maintaining accurate records and adhering to statutory guidelines.
* Supporting individuals in both community and hospital settings.
Requirements:
* Fluency in Welsh (essential).
* Experience in community care and hospital discharge processes (desirable).
* Strong communication, assessment, and organizational skills.
* A commitment to enhancing the well-being of older adults.
