We provide excellent primary care and innovative services to a diverse city centre patient population of around 9,000 patients.Main duties of the jobIn accordance with the Practice timetable, the post-holder will make him/herself available to undertake a variety of duties including:Surgery consultationsTelephone consultations and queriesVisiting patients at homeChecking and signing repeat prescriptionsDealing with queries, paperwork, reports, and correspondence in a timely fashionMaking professional, autonomous decisions regarding presenting problemsAssessing healthcare needs of patients with undifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signs of illnessDeveloping care plans in consultation with patients, following current Practice protocolsProviding counselling and health educationAdmitting or discharging patients to/from the caseload and referring to other care providers as appropriateRecording clear and contemporaneous consultation notesCollecting data for audits and complying with QOF requirementsDelivering enhanced servicesCompiling and issuing computer-generated prescriptionsPrescribing in accordance with the Practice formulary or generically when appropriateThe post-holder will be expected to undertake all normal duties and responsibilities associated with the role.About usOur practice is supportive and values its dedicated team, which includes four GP Partners, two Salaried GPs, a Physician Associate, Clinical Pharmacist, Physio, Practice Nurses, Healthcare Assistant, and our management and admin team. We are active members of our Primary Care network, working with people experiencing homelessness and tackling health inequalities.We offer four to six clinical sessions, with flexibility for the right candidate. We are an active training practice with GP registrars and have a strong tradition of postgraduate medical education, including opportunities to develop teaching skills. We also have attached medical students.As a research-active practice, applicants can develop research skills. We have strong links with the Deep End network, serving the most socioeconomically deprived areas of Yorkshire and the Humber, passionate about addressing health inequity.
