Salary £13.89 per hour (£14.89 weekend rate) plus a £500 Welcome Bonus.

About the Role:
The D2A (Discharge to Assess) service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home. Your responsibilities will include:
* Writing support plans following the initial visit, ensuring person-centred care is provided.
* Motivating customers, building their confidence so they can achieve their goals and return to independent living.
* Supporting customers with tasks around the home and with personal care.
* Working a mixture of early and late shifts between 7 am and 10 pm, including alternate weekends.
* Being on-call on a roster basis, which attracts an additional payment.
About you:
* You will hold (or be working towards) a QCF Level 3 in Health and Social Care.
* Previous experience in caring for vulnerable adults with complex needs is essential.
* You are sensitive, caring, and passionate about delivering a first-class service.
* A full driving licence and access to your own vehicle are required, as you will need to travel between clients across Sedgemoor, including Burnham, Cheddar, and Bridgwater.
* You will support clients across the whole of Sedgemoor, with pay per shift including travel time.
Additional benefits include:
* £500 welcome bonus on successful completion of probation.
* Double time on bank holidays and an extra £1 per hour on weekends.
* Mileage allowance of 45p per mile.
* Paid annual leave: 30 days including bank holidays, increasing to 33 after 5 years of service (pro-rata).

Our D2A team covers Bridgwater, Highbridge, Burnham-On-Sea, Cheddar, and surrounding areas, supporting clients discharged from hospital to recover at home.
