Overview
Advanced Nurse/Clinical Practitioner – 24 hours per week. Based at a GP practice in Abbeydale, Gloucester. Part-time role with an independent prescriber and autonomous clinical responsibilities within an urgent care team.
Responsibilities
* Deliver a high standard of urgent care to patients using autonomous clinical skills and be an independent prescriber.
* Provide a holistic approach to patient care including assessment, diagnosis, planning and implementation of high-quality care.
* Work within the urgent care team alongside other AHPs/ACP and GPs; ensure patients are seen by the right person at the right time.
* Manage own clinical workload and develop referral pathways to local NHS Trust services and Diagnostic and Treatment Centres.
* Mentor and support other healthcare professionals in developing and maintaining clinical skills.
Main duties & Areas of responsibility
* Work alongside on-call GPs; provide telephone triage and on-the-day appointments as appropriate.
* Act as first point of contact for undifferentiated, undiagnosed problems with history taking, examination, problem solving and clinical decision making.
* Provide direct access to NP/ACP in the practice and over the telephone for the population served.
* Prescribe safely and appropriately within current legislation; conduct necessary investigations and interpret results.
* Offer evidence-based, cost-effective patient care and develop ongoing care plans with emphasis on prevention and self-care.
* Promote health through education, screening and therapeutic communication; refer to other services as appropriate.
* Contribute to meeting National Service Frameworks and work with multidisciplinary teams to deliver integrated care.
* Participate in identifying community health needs and developing patient-centered strategies.
* Support the development of new patient services and quality improvements; contribute to GP commissioning planning.
Qualifications & Experience
* Registered General Nurse with current NMC registration.
* Recognised NP/ACP qualification at degree level or equivalent.
* Independent Nurse Prescriber.
* Minimum of 5 years post-registration experience with evidence of CPD and autonomous working in NP/ACP roles.
* Demonstrated ability to work effectively within a team and evaluate the safety and effectiveness of clinical practice.
Skills
* Knowledge of policy developments related to primary care delivery, GMS contract, Clinical Governance and NICE guidance.
* Strong assessment, decision-making, and risk management skills; excellent communication and IT skills.
* Leadership and ability to promote the NP/ACP role within the practice and externally.

We offer a supportive, well-managed working environment with a happy and functional team and have a £5M building to meet the clinical needs of our patient population. We are a SystmOne practice employing around 90 staff including GPs, ANPs, Paramedics, Practice Nurses, Mental Health Practitioners, Physiotherapists, HCAs, Patient Advisors and Administrators.
