Overview
We are part of the South Tees Nutrition and Dietetic service working across The James Cook University Hospital, Middlesbrough, Redcar & Cleveland community, Friarage Hospital Northallerton and Hambleton & Richmond community.
Working within defined scope of practice and service guidelines, key aspects of this role will include:
o assessing the nutritional needs of non-complex adult patients with a range of medical conditions, o devising programmes of dietetic care under dietetic supervision, o monitoring and evaluating the effectiveness of dietary intervention and providing regular feedback to the supervising dietitian, o Supporting team with the discharge and ongoing administrative management of patients who are fed via a feeding tube in the community, o Liaising with dietitians, community staff, home enteral feeding company, ward staff daily
Responsibilities
* Assessing the nutritional needs of non-complex adult patients with a range of medical conditions
* Devising programmes of dietetic care under dietetic supervision
* Monitoring and evaluating the effectiveness of dietary intervention and providing regular feedback to the supervising dietitian
* Supporting the team with the discharge and ongoing administrative management of patients who are fed via a feeding tube in the community
* Liaising with dietitians, community staff, home enteral feeding company, ward staff daily
Qualifications
* Enthusiastic and organised person with excellent communication skills
* Strong and confident communication skills when liaising with patients, carers, relatives and other health care professionals
* Knowledge and interest in nutrition through qualifications and/or relevant experience
* Relevant qualification required with demonstrated nutrition knowledge, excellent communication and organisational skills
Requirements
This post will be based at our community site at Langbaurgh House, Guisborough. You will need to have access to a vehicle for work purposes to meet the requirements of the role. The role supports an already established team of dietitians and discharge coordinator who manage patients who are tube fed at home. Full training will be provided.
Training & Development
