Our Thame Rapid Response and Intermediate Care teams are busy places to work and you will be using your skills to support a caseload of patients with both acute and chronic conditions enabling them to maximise their independence. The role has been developed to support the clinical case managers and you will be working alongside Physiotherapists, Occupational Therapists, Nurses and Paramedics as well as rehabilitation focused healthcare assistants. You will carry your own caseload as well as supporting and reviewing people receiving services from the team.
Main duties of the job
1. Deliver clinical and therapeutic care, within the clinical setting within set protocols and within the scope of the Assistant Practitioner.
2. Perform patient assessment, plan and deliver high standards of care. Develop, implement and evaluate individual care/ treatment plans (in accordance with proven competency and after appropriate delegation from a registered practitioner).
3. Be responsible for carrying a caseload of non-complex patients.
4. Co-ordinate and undertake the safe admission, transfer and discharge of patients in accordance with Trust policies. Perform safe, effective and timely discharge planning.
5. Assist patients in maintaining their personal hygiene, including oral hygiene, grooming and dressing needs with respects for culture and religious needs, ensuring privacy and dignity at all times. Enable and assist the patient as directed or outlined within the patient’s goal planning progress plan appropriate to their ability and condition.
6. To be responsible for safe and competent use of all equipment/ mobility aids.
7. To promptly report any equipment failures or accidents to the Therapy staff and other appropriate members of staff, following the correct departmental procedures.

Skills, Abilities & Knowledge
Essential criteria
18. General computer skills
19. Ability to learn / ability to self-initiate learning (evidence within the last 2 years).
20. Good knowledge of the audit process.
21. Collaborate with multi-professional service users, internal and external at all levels.
22. Able to motivate self and be able to work with all levels of staff.
23. Able to accept and undertake responsibility for designated tasks.
24. Clear, concise verbal and written communication to ensure liaison with multi-disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English).
25. Ability to learn/ability to self-initiate learning (evidence within the last 2 years).
26. Awareness of Health & Safety issues and policies.
27. Able to know own limitation and seek advice.
Experience
Essential criteria
28. Significant experience working within a patient facing environment with a reablement / rehabilitation focus
29. Previous work-based experience of patients with physical, cognitive or behavioural impairment
Education, Qualifications & Training
Essential criteria
30. Commitment to undertaking Foundation Degree and any associated training once available
31. Level 3 Health and Social Care Diploma or equivalent
