Hours: Full-Time (37.5 hours)

Main duties of the job
* To work as part of a team ensuring all referrals are triaged and visits scheduled promptly.
* To ensure all new admissions to Community Nursing services have a comprehensive assessment and care plan.
* This role covers the entire borough.
* To promote self-management and self-care among patients.
* To ensure safe handovers and effective care plan implementation with measurable outcomes.
* To collaborate with locality teams and partner providers to maintain effective patient flow.
* To work within the allocated financial resources.
* To work on a rota with shifts from 8am-4pm, 10am-6pm, and 12pm-8pm, Monday to Sunday.

Education/qualification/training
* RGN
* DN Cert/Dip/Degree
* Advanced history taking and physical assessment course
* Non-Medical Prescriber V300
* Nurse Formulary prescriber V100
Experience
* Ability to deliver high standards of evidence-based clinical nursing care
* Ability to assess, monitor, plan, and evaluate care
* Good organizational skills and ability to prioritize and delegate effectively
* Effective liaison with primary care team members
* Experience with holistic initial assessments
* Experience managing a healthcare team
Knowledge and skills
* Understanding of complex needs and long-term condition management
* Knowledge of safeguarding processes and escalation procedures for vulnerable adults
* Competence in IT and clinical systems like Emis
* Knowledge of community nursing practice and legislation
* Understanding of information governance
* Knowledge of HR processes for staff management
* Awareness of current nursing professional issues
