An exciting opportunity has arisen for a full-time, 37.5 hrs band 6 Mental Health Liaison Practitioner to join the Mental Health Liaison team based in Princess of Wales hospital.
The Mental Health Liaison Team is set up to provide a service for older adults admitted to the general hospital as well as the emergency departments. The liaison team covers the Bridgend area out-reach service for care homes within the Bridgend Area.
The team offers timely comprehensive assessment of the mental health needs of those referred that present with unexplained medical conditions that may be a consequence of mental health illness or co-occurring mental health and physical health problems and individuals that are presenting with incidents of self-harm.
The successful candidate will possess excellent communication skills, be a good team player with a flexible attitude, and the ability to provide high-quality individualised assessments and advice on the care and treatment of patients within the general hospital setting.
Work effectively within a multi-disciplinary team and demonstrate a genuine interest and enthusiasm for caring for people with mental health difficulties.
The main purpose of this role is to contribute to a 5-day a week provision of a Mental Health Liaison Service. May have to cover out of hours for the older persons community service on a rota system.
You will need to demonstrate a sound clinical knowledge and awareness of current clinical practice and have sound knowledge of safeguarding procedures, the Mental Health Act, Mental Capacity Act, and the Mental Health Measures.
The post holder must have good communication, listening, and organisational skills which are essential for the role along with excellent clinical assessment and formulation skills to meet the needs of the clinical team.
The post holder must have a positive attitude, be able to work as a member of a team and autonomously in a challenging and changeable environment, and have excellent presentation, oral, and written communication skills always acting in a professional manner.
Provide training and education for both Pre-Registration and Post-Registration nursing students and other health care professionals/agencies.
Contribute to the development of a comprehensive liaison service.
Foster robust working relationships with colleagues from all disciplines within the general hospital setting and within the mental health services to facilitate improved joint working.
