Job summary We are seeking an enthusiastic individual to take on therole of Practice Nurse. The role is to ensure the delivery of an effectivePractice Nurse service to the practice population, delivering careappropriately according to practice policy, protocols, guidelines andprocedures. Main duties of the job To carry out a wide range of treatment room duties e.g: venepuncture, new registration checks, blood pressure checks, ECGs, vaccinations and immunisations To run Chronic Disease Clinics such as CHD, Diabetes and Respiratory. Offer appropriate lifestyle advice to patients. To provide support to the nursing team to ensure smooth running of the nursing service. Practice Population Circa 11,000 Patients. Friendly and passionate team of 5 GPs and 3 Advanced Nurse Practitioners. Excellent management and well-established support team to ensure the practice is well organised and structured. Friendly, supportive atmosphere. Practice uses SystemOne practice software with Ardens templates Dedicated pharmacy team who handles all medication related queries and repeat prescriptions Dedicated in-house PCN staff including CPs, ACPs, Mental Health Workers, Care Coordinators and Health and Well-Being Coaches Dedicated Community and Home Visiting Teams Large, modern, purpose-built surgery with excellent facilities. Job description Job responsibilities Clinical Practice: Assess, plan, provide and evaluatetreatment programmes including, but not limited to: Manage and treat patients with long term conditions in linewith national / local policies and practice needs with the inclusion of Home Visits Review medication for therapeutic effectiveness inaccordance with evidence based practice and national / practice protocols, andwork with patients to support adherence to prescribed treatments Support and manage health needs of women presenting for Family Planning and Cervical Cytology Consultations Implement and participate in Vaccination and Immunisation Programmes for both Adults and Children Confidentiality In thecourse of seeking treatment, patients entrust us with, or allow us to gather,sensitive information in relation to their health and other matters. They do soin confidence and have the right to expect that staff will respect theirprivacy and act appropriately In theperformance of the duties outlined in this job description, the post-holder mayhave access to confidential information relating to patients and their carers,practice staff and other healthcare workers. They may also have access toinformation relating to the practice as a business organisation. All suchinformation from any source is to be regarded as strictly confidential Informationrelating to patients, carers, colleagues, other healthcare workers or thebusiness of the practice may only be divulged to authorised persons inaccordance with the practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data Health & Safety The post-holder will implement and lead on a full range ofpromotion and management their own and others health and safety and infectioncontrol as defined in the practice Health & Safety policy, the practiceHealth & Safety manual, and the practice Infection Control policy andpublished procedures. This will include (but will not be limited to): Usingpersonal security systems within the workplace according to practice guidelines Awarenessof national standards of infection control and cleanliness and regulatory /contractual / professional requirements, and good practice guidelines Responsiblefor the correct and safe management of the specimens process, includingcollection, labelling, handling, use of correct and clean containers, storageand transport arrangements Responsiblefor hand hygiene across the practice Identifyingthe risks involved in work activities and undertaking such activities in a waythat manages those risks across clinical and patient process Safemanagement of sharps procedures including training, use, storage and disposal Usingappropriate infection control procedures, maintaining work areas in a tidy,clean and sterile, and safe way, free from hazards. Initiation of remedial /corrective action where needed or escalation to responsible management Activelyidentifying, reporting, and correction of health and safety hazards andinfection hazards immediately when recognised Keepingown work areas and general / patient areas generally clean, sterile,identifying issues and hazards / risks in relation to other work areas withinthe business, and assuming responsibility in the maintenance of generalstandards of cleanliness across the business in consultation (whereappropriate) with other sector managers Undertakingperiodic infection control training Spillagecontrol procedures, management and training Decontaminationcontrol procedures, management and training, and equipment maintenance Maintenanceof sterile environments Demonstratedue regard for safeguarding and promoting the welfare of children.
