What you will do:
* Lead the management and ongoing care of patients with MND
* Conduct advanced assessments and plan and review treatments
* Coordinate multidisciplinary team working across neurology, respiratory, speech and language therapy, nutrition, and palliative care
* Provide home visits, hospital reviews, and run virtual and face-to-face clinics
* Support advanced care planning, investigations, and clinical decision-making
* Attend monthly MND multidisciplinary team meetings at the local hospice
* Support service development, research, audit, and implement best practice guidance
* Advanced clinical assessment skills
* Experience caring for patients with neurological disorders, respiratory failure, nutritional or swallowing issues, and progressive neurological conditions
* Evidence of managing neurological patients in community and hospital settings
* Full driving licence and access to a car
* Master’s degree (completed or in progress)
* Respiratory care, including non‑invasive ventilation management
* Recent experience supporting patients with MND
