Job responsibilities
You will
* Conduct consultations This includes face to face, telephone and home visitassessments where required.
* Manage acute and chronicconditionsFrom urgent presentations to longterm disease management,ensuring continuity of care.
* Prescribing and medicationreviews Safe, evidence basedprescribing and regular review of repeat medications.
* Referrals and investigationsOrderingtests, interpreting results, and referring patients to secondary care orcommunity services
* Maintain accurate clinicalrecordsUsing practice systems to document consultations, results, andcare plans.
* Contribute to practice targetsand QI Supporting QI, enhanced services, audits, and quality improvementinitiatives.
* Support practice governanceParticipating in clinical meetings,significant event reviews, and safeguarding processes.
* Work within the GMS contractframework
* Collaborate with themultidisciplinary teamWorking closely with partners, nurses,pharmacists, and administrative staff to ensure smooth service delivery.
