Social Work Assistant - Central Locality Team (Adults)

Location: High Wycombe

We have a fantastic opportunity for a dynamic Social Work Assistant who is passionate about enabling clients to fulfill their potential while promoting self-determination to join our Central Locality Team.

If you have experience in Health and Social Care, assessing needs, a desire to help clients achieve or maintain independence, and enjoy working in a fast-paced environment, we want to hear from you!


About us

Buckinghamshire Council is a relatively new organisation that combines the strengths and expertise of the previous five councils in the county to create positive change for local people, communities, and businesses. About the role

You will assess and review the needs of clients referred for or already receiving care and support services, producing high-quality assessments, reviews, and care plans focused on independence and individual outcomes.

You will collaborate with other professionals and organisations to support residents holistically. Social Work Assistants will endorse and act according to personalisation principles, ensuring care plans are person-centred and, as far as safely possible, empowering individuals to control their lives.

This 'mobile' role offers flexibility to schedule your own diary, meeting clients and colleagues in various locations. You will access digital resources via your laptop and work remotely, with occasional meetings in the office.


About you

We seek a passionate, supportive, motivated, and confident individual who communicates well both on the phone and face-to-face.

Your responsibilities include carrying out needs assessments, creating person-centred care plans, ensuring service delivery, and reviewing outcomes with clients, their networks, and providers.

You should demonstrate emotional resilience and intelligence—maintaining focus and perseverance despite setbacks and stress.
