Overview
An excellent opportunity has arisen for a Band 6 Anticoagulation Nurse to work as a member of the Anticoagulation Team within the Pathology Department. You will carry out the analysis and interpretation of technical investigations of clinical samples to dose patients according to the current guidelines for anticoagulation.
Responsibilities
* Clinical assessment of patients – indications for therapy, the therapeutic ranges and necessary duration of therapy.
* Undertake Anticoagulation clinics in multiple settings including the hospital, community and patients’ homes.
* Monitor and dose patients in the Anticoagulation clinics and in patients’ own homes and refer to other clinicians or services as needed.
* Educate newly referred patients and their carers in aspects of anticoagulation management and monitoring who attend inpatients and community-based clinics.
* Carry out diagnostic testing using Near Patient testing devices such as a CoaguChek or similar to obtain INR results.
* Prescribe anticoagulation therapy in accordance with local guidelines and following appropriate assessment of patients INR or creatinine clearance status.
* Initiate oral anticoagulation for differing conditions and ensure correct bridging with LMWH when circumstances arise.
* Offer advice and information on request on issues associated with anticoagulation management and monitoring from other clinicians, patients and carers.
* Undertake phlebotomy as required to obtain venous blood samples.
* Contribute to influencing all aspects of patient care and management within the specialty and promote the delivery of high quality, clinically effective care, prioritising and utilising research in everyday practice providing specialist advice to patients.
Qualifications and Experience
* Registered Nurse of the appropriate part on the NMC Register
* NMP Qualification
* Proficient in Phlebotomy
* Previous experience within an anticoagulation setting
* Excellent communication and interpersonal skills
* Driver prepared to travel throughout the borough of Walsall
