What will I be doing?
* Complete Care Act assessments, reviews, mental capacity assessments, safeguarding enquiries, court of protection work.
* Participate in a duty system across the county.
* Use motivational interviewing skills, crisis intervention, and positive risk management.
* Collaborate with colleagues and specialist professionals in weekly Peer Forum discussions.
* Engage with specialist teams such as Preparing for Adulthood, Commissioning, Occupational Therapy, DOLS, Mental Health, Safeguarding, CHC, and LD Health colleagues in Somerset Foundation Trust.
* Advocate using a strength-based, personalised approach under the three conversations model.
What kind of experience or qualifications do I need?
* Qualified Social Worker and registered with Social Work England.
* Full access to a vehicle for travel to any location within Somerset.
* Ability to travel across a largely rural county; reasonable adjustments available for disabled individuals.
* Police/DBS clearance via Disclosure procedure.
