Offer description

The Band 6 CYP Continuing Care Assessor is responsible for carrying out high-quality assessments for children and young people with complex, ongoing health needs, ensuring eligibility decisions are fair, transparent, and aligned with the national Children & Young People's Continuing Care Framework. The postholder works directly with families and multidisciplinary partners to build a clear, holistic picture of each child's needs and contribute to safe, person-centred care planning. This is an operational clinical role that requires strong assessment skills, sound professional judgement, and the ability to manage sensitive situations with confidence and empathy. The Assessor gathers and interprets evidence, contributes to Decision Support Tools or equivalent frameworks, and produces clear recommendations that support robust and defensible decision-making. The CYPCC Assessor works closely with health, social care, education, SEND, and safeguarding colleagues, helping ensure assessments are well coordinated and that families receive accurate, timely communication throughout the process. They manage a defined caseload and escalate complex or high‑risk situations where senior clinical input is required.

We are currently looking for 6 WTE CYPCC Nurse Assessors to work across the Central East area which includes Cambridge and Peterborough, Bedfordshire and Hertfordshire.
