Job summary

This is a great opportunity to build your clinical and leadership skills while supporting people with complex health needs in the community. You'll work as part of an experienced community nursing team to provide personalised care for people living with long-term conditions. Your work will help people stay well at home, avoid unnecessary hospital admissions, and feel confident in managing their health. Working closely with patients, carers and partners across health and social care, you'll play an important role in shaping safe, coordinated, and responsive care.


Main duties of the job

What you'll do:

* Complete holistic assessments and create care plans that meet individual needs
* Act as a care coordinator, supporting smooth transitions and effective communication
* Deliver a range of clinical interventions such as wound care, IV therapies and end-of-life support
* Monitor caseloads proactively and respond to changes in patients' conditions
* Maintain accurate, up-to-date electronic care records
What we're looking for:
* A registered nurse with up-to-date clinical knowledge and evidence of ongoing development
* Experience of community nursing or caring for people with long-term conditions
* Strong communication skills and confidence working with patients, families and partners
* Ability to work independently, prioritise work and support colleagues through supervision
* Willingness to work towards non-medical prescribing if not already achieved

* RGN/RN/Diploma Nursing Studies
* Non-medical prescribing V100/V150 or willing to work towards within specified timeframe
* Specialist training/short courses relevant to the post or equivalent level of experience
* Evidence of ongoing continuing professional development/Portfolio


Knowledge


Essential

* Current Legislation and impact on community nursing services
* Evidence based practice
* Mentorship or evidence of supervision/teaching/team management
* Evidence of understanding of the role of Case Manager and care coordination
* To demonstrate an understanding of proactive caseload management
