1. Primary Duties and Areas of Responsibility
1. Liaise with GPs regarding the practice palliative care register, promoting the use of the EARLY clinical search tool to proactively identify those who may be in the last 12 months of their life but not on the palliative care register.
2. Support the clinical validation process and review, where clinically appropriate, those who may benefit from an advance care planning discussion.
3. Ensure correct documentation and coding via EMIS to facilitate information sharing through EPACCS and other modes.
4. Act as a core member of the PCN MDT, liaising with providers of palliative and end-of-life care to refer or signpost to appropriate services.
5. Coordinate palliative care register meetings, ensuring equitable access to care and GP review for those with complex needs.
6. Utilise communication skills for holistic assessment of individuals’ physical, psychosocial, emotional, and spiritual needs, eliciting their preferences and wishes.
7. Develop advance care plans, prioritising the individual and their loved ones in future care decisions.
8. Lead on collecting data, developing quality improvement plans, and reporting to monitor progress, recognising inequalities in care.
9. Audit and report on key deliverables and progress of quality initiatives, sharing best practices across Lancashire & South Cumbria.
10. Contribute to the management and development of the ACPP role and align with the National Palliative Care Agenda.
2. Education, Training and Development
1. Engage in continuous professional development and collaborate with clinical pharmacists to identify development areas.
2. Participate in delivering formal education programs.
3. Stay updated with current educational policies and clinical practices.
4. Participate in staff surveys and audits as required.
5. Review and implement new working methods to optimize system use.
6. Ensure team competencies are maintained to provide quality services.

1. Comply with health and safety policies, report incidents via the designated system.

1. Adhere to infection prevention policies and complete mandatory training.

1. Maintain confidentiality in accordance with Data Protection Act 1998 and related policies.

1. Travel independently between sites and attend meetings hosted by other agencies.
2. Handle contact with body fluids during clinical practice.
