East Cheshire NHS Trust has an exciting opportunity for a dynamic associate practitioner to join our Macclesfield Care Community therapy team.

We welcome applications from motivated individuals with a keen interest in working within an integrated team, with proven experience in community rehabilitation or general in patient rehabilitation.

Please be advised we are unable to provide Skilled Worker Visa Sponsorship to non-UK residents.


Main duties of the job

To work as part of a small therapy team providing a comprehensive, rehabilitation service for patients with a broad range of complex conditions.

You will be responsible for the delivery of therapy independently or jointly with the therapists in patients own homes, residential and nursing homes.

To contribute to multidisciplinary team management.

To contribute to operational delivery and service development.

The community therapy service is operational over 7 days, from 8am-8pm. You may occasionally be required to work a variety of shift patterns across the 7 day service, to meet service need.

Person specification


Qualifications

* 5 GCSE’s including English and Maths or equivalent
* BTEC National Diploma or equivalent in a relevant subject


Specfic Job requirement

* Skills in delivering therapy programmes
* Ability to adapt programmes to meet the changing needs of the clients with reference to the supervising therapist
* Ability to support and motivate patients as appropriate
* Ability to carry out and complete delegated project
* Experience of contributing to audit
* Experience of working with the patients following stroke and/or neurological conditions
* Evidence of working as a therapy assistant
* Ability to contribute to patient assessment
* Ability to organise therapy programmes in consultation with the responsible therapist


Experience

* Experience of contributing to individual & group interventions for patients & settings relevant to the role
* Experience of contributing to audit
* Experience of working with the relevant specialist patient group
* Evidence of working as a therapy assistant


Skills

* Effective communication and listening skills
* Takes own initiative and seeks appropriate guidance
* Ability to work as part of a team
* Ability to teach practical skills
* Good organisational skills


Knowledge

* Knowledge / competency of a range of assessment tools & interventions relevant to the role
* An understanding of confidentiality & Information Governance
* Knowledge of relevant terminology and clinical conditions
