Clinical Nurse Specialist TWOC (Trial without Catheter) Triage

Following the success of the TWOC Improvement Project, we are delighted to offer a substantive Band 6 Clinical Nurse Specialist post within our ambulatory clinic services.

As the TWOC Triage CNS, you will play a primary role in assessing, triaging and coordinating care for patients undergoing TWOC, ensuring timely, safe, and effective decision-making.

We are looking for a proactive, experienced triage nurse with strong clinical judgement, excellent communication skills, and a collaborative approach. If you're passionate about improving patient experience and want to be part of a service thats making a real difference, wed love to hear from you.

In This Role, You Will

Use advanced clinical evaluation skills to assess and triage patients

Communicate effectively with waiting list coordinators, ambulatory clinic teams, and urology services across the Health Board

Provide timely, safe, and efficient triage decisions to support patient flow and service delivery

Take responsibility for collating and reporting TWOC triage data on a monthly basis

Contribute to the ongoing development of this evolving and impactful service

Work base to be confirmed in agreement with the successful candidate.

Accept and triage referrals for patients requiring a Trial Without Catheter (TWOC), ensuring they meet clinical criteria and are prioritised appropriately.

Review referral documentation and liaise with referring clinicians to clarify clinical information where necessary.

Coordinate and manage the TWOC triage pathway, ensuring timely scheduling and follow-up of patients.

Conduct pre-assessment of patients, including reviewing medical history, current catheter use, and suitability for TWOC.

Provide expert clinical assessment and decision-making regarding patient readiness for TWOC.

Possess a high standard of knowledge regarding catheter management and Trial without Catheter procedures.

Deliver training to departments regarding TWOC process.

Deliver patient-centred care and education, including information on the TWOC process, potential outcomes, and self-care.

Maintain accurate and timely documentation of all triage decisions, assessments, and patient outcomes.

Communicate effectively with multidisciplinary teams to ensure continuity of care and appropriate escalation where needed.

Participate in service audits, data collection, and quality improvement initiatives related to continence and urology services.

Support the development and implementation of clinical guidelines and pathways for TWOC management.

Provide mentorship and support to junior staff.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our Services Are Provided In

Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest

Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire

Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings

47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

Numerous mental health and learning disabilities services

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
