We are looking for Clinical Homecare Nurses to join our nursing team in providing professional services to our clients, patients and healthcare providers.

Main Responsibilities:
* Be part of an innovative team committed to delivering safe, high-quality nursing services in the home.
* Assist in providing Patient Support Programmes and develop, implement, and deliver Clinical Services within the community.
* Ensure high-quality service delivery, meeting KPIs and business requirements.
* Provide excellent patient care on behalf of clients and healthcare professionals.
* Teach patients/carers to self-administer therapy or administer therapies according to protocols, including via subcutaneous, intramuscular, or intravenous routes.
* Participate in telephone, virtual, and community visits as part of Patient Support Programmes.
* Engage in Clinical Governance activities, including supervision.
* Support seamless service delivery between hospital and home, promoting treatment adherence and enhancing quality of life.
* Accurately record all patient care activities following SOPs and regulatory guidelines.
* Maintain professional competence and current nursing practices in line with The NMC code.
* Engage in Continuing Professional Development and share knowledge with the team.
* Ensure high standards of service for all customers and service users.
* Record, report, and act upon patient information, communicating changes to relevant healthcare professionals.
* Maintain medical and ancillary supplies and ensure equipment is in good working order.
* Contribute to a safe workplace environment for staff, patients, and visitors.
* Promote positive staff morale and performance.
* Assist with induction of new staff as needed.
* Support development of customer relationships with clients.
* Assist/support the Commercial Team and other team members in patient care and customer interactions.
* Maintain adequate stock of nursing supplies for patient visits, ensuring items are in date and correct.
* Support the safe and effective implementation of new business opportunities.
Clinical Services Responsibilities (if applicable)
Support NHS clinics to enhance patient services by providing additional resources for seamless transition to Homecare, including:
* Regular clinic attendance as agreed.
* Assist in introducing patients to Homecare concepts.
* Provide patient information for Homecare initiation.
* Administer medication as required.
* Train patients or carers in self-administration of medication.
* Record all documentation accurately.
* Complete additional administrative tasks as needed.
