We offer an exciting opportunity for a Band 3 Community Healthcare Support Worker to join our innovative Community Frailty Virtual Ward, delivering high‑quality, personalised care to adults living with frailty across Plymouth, the South Hams and West Devon.

Responsibilities
* Provide direct care to patients in their own homes, including personal care, mobility, manual handling and daily living activities.
* Take clinical observations, conduct ECGs, venepuncture and bladder scanning, and support clinical teams with information gathering and ongoing monitoring of frailty‑related symptoms.
* Work closely with Frailty Practitioners, Clinical Nurse Specialists, Frailty Nurses, Therapists and other medical professionals to ensure coordinated, patient‑centred care.
* Contribute to early identification of deterioration and escalation where needed.
* Promote independence, dignity and safe self‑management.
* Maintain accurate, confidential patient records and report changes in condition to registered staff.
* Participate in assessments, discharge planning, care planning and reviews.
* Support therapeutic activities and record interventions electronically.
* Assist with clinical supplies and stock levels, and travel across Plymouth when required.
* Complete breakaway training and carry a personal alarm in community settings.
Qualifications
* NVQ Level 3 in Health and Social Care or equivalent.
* Care Certificate or willingness to undertake.
* Additional training relevant to physical healthcare (desirable).
* Vocational Qualification Level 2 (desirable).
Experience
* Previous healthcare experience, preferably in a physical health setting.
* Experience supporting vulnerable adults and working with minimal supervision.
* Experience with baseline observations and record keeping.
* Experience in multidisciplinary team working and supporting recovery goals.
* NHS experience (desirable).
* Driving licence for community roles (desirable).
