We have an exciting opportunity for a Specialist Physiotherapist to join the Community Division in the borough of Wigan. We are aiming to recruit a suitably qualified, enthusiastic and engaged individual into our Community Therapy Team. The post holder will work within a multi-disciplinary team to manage a defined caseload. They will provide input to patients at home and within the community. The post holder will be able to provide senior support and supervision to other staff members, participate in triage and be supportive to the teams leads in the service. The successful candidate will work within a multidisciplinary team, supporting the provision of high quality patient centred health care. The ideal candidate will be innovative, have excellent communication skills, and be willing to work flexibly to support the development of the service. This post is permanent full time (37.5 hours) worked over Monday to Friday. Enhanced DBS with Adults will be required which will be obtained during pre-employment checks. Significant Band 5 NHS experience is essential. As the role involves visiting patient homes & clinics across the Trust every day, postholders must state in their application if they have access to a car with suitable business insurance. Main duties of the job

* To complete functional and specialist physiotherapy assessments of people in their own homes and community settings.
* To plan and implement effective interventions, working closely with other community services, to overcome identified functional barriers while effectively managing risk.
* To act as a source of knowledge on the management of a varied range of conditions, providing an advisory service to patients, relatives and health and social care professionals.
* On occasion work in residential homes (respite) in order to identify barriers to returning to their usual place of residence.
* To assist the operational and clinical leads in planning, coordinating, delivering and evaluating the service provided on a day-to-day basis.
* Function as an effective member of the multi-disciplinary team.
* To manage a defined caseload using evidence based/client centred principles to plan, implement and evaluate interventions.
* To regularly supervise students on practice placement.
* To provide leadership and clinical supervision for other junior staff.
* To consider and support service development alongside senior colleagues including data collection, audit, service review etc.

Duties and Responsibilities

Key results from the job holder

* To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure high standards of clinical care for the patients under your management, and support more junior staff to do likewise.
* Post holders are required to work independently, guided by defined clinical and professional guidelines. Access to a more senior member of physiotherapy staff is available when necessary. Clinical work is evaluated on a regular basis.
* To interpret and analyse clinical and non-clinical facts to form accurate assessment and risk stratification in a wide range of conditions, to recommend the best course of intervention, and to develop comprehensive discharge plans in regard to the patient’s self-management.
* To undertake the comprehensive assessment of patients, including those with a complex presentation, using investigative and analytical skills and to formulate individualised management and treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options to formulate a specialised programme of care.
* To maintain accurate and comprehensive patient treatment records in line with Trust Policies.
* To maintain timely statistics and other records as requested by the Community Therapy Service and the Trust.
* To regularly supervise, evaluate and assess Physiotherapy students to include negotiating learning contracts and setting objectives. To monitor the quality of record keeping for students.
* To comply with The Chartered Society of Physiotherapy Code of Ethics and Professional Conduct be registered with the Health & Care Professions Council and Trust Policies and Procedures.
* To respect the individuality, values, cultural and religious diversity of clients and contribute to the provision of a service sensitive to these needs.

Planning and Organisational Duties

* To accept clinical responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time.
* To accept partial responsibility for any tasks delegated to less senior qualified or unqualified staff.
* To assist the team leaders in developing policy changes within the speciality that will impact on service users. To assist the team leaders in ensuring that designated staff implement policy and service development changes.
* To work with the team leaders in developing the operational management of the Community Therapy Service.
* To participate in the day-to-day organisation and development of the Service.
* To contribute to the Trust’s clinical governance arrangements and quality agenda.
* To demonstrate understanding of national and local guidelines and legislation relating to health and social care and their impact on service delivery.
* Be responsible for the monitoring of materials and equipment and bring defects or deficits to the attention of the Clinical Manager.
* To exercise good personal time management, punctuality and consistent reliable attendance.
* To review and reflect on own practice and performance through effective use of professional and operational supervision and appraisal.
* To maintain a professional portfolio of Continuing Professional Development recording learning outcomes through participation in internal and external development opportunities.
* To comply with annual PDR and appraisal system.
* To contribute to development of practice/procedures etc. within the Community Therapy service

Communications and Key Working Relationships

* To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of condition.
* To use communication tools (verbal and non-verbal) to gain informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, anxiety.
* To assess capacity, gain valid informed consent and have the ability to work within a legal framework.
* To assist the Team leaders in ensuring that designated staff implement policy and service development changes.
* To communicate complex patient related information and non- patient related information effectively and work collaboratively with medical and nursing and therapy colleagues to ensure a coordinated approach to care.

Responsibility for Finance

* To advise Clinical Manager on resources required to carry out the job.

Responsibility for Human Resources

* To assist the team leaders to educate, supervise and performance manage more junior staff, therapy assistants, and students. Including formal supervision and appraisal.

Responsibility for Health & Safety

* Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Responsibility for Teaching

* To provide spontaneous and planned advice, teaching and instruction to relatives, carers, and other professionals, to promote understanding of the aims of physiotherapy, and to ensure a consistent approach to patient care.
* Supervise, educate, and assess the performance of physiotherapy students; this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification.

Qualifications

* Degree in Physiotherapy
* HCPC Registered Physiotherapist
* Evidence of post graduate courses and CPD
* Courses in Falls or community
* Clinical educator certificate
* Relevant clinical courses
* Member of relevant specialist interest group
* Member of the CSP


Experience

* Varied experience in working with the elderly and Falls
* NHS experience
* Clinical experience working in areas of intermediate care, community and falls or similar.


Skills

* Able to communicate with individuals and groups at different levels and variety of settings
* Familiar with a variety of outcome measures and interventions used within the Falls and community setting
* Ability to work independently
* Able to work collaboratively within the multidisciplinary team
* Able to maintain judgement under pressure
* Negotiation and motivational interviewing skills.


Knowledge

* Knowledge of Health and Social Care Integration
* Knowledge of a wide range of conditions and their physiotherapy management
* Advanced clinical knowledge in Falls
* Knowledge of the principals of Clinical Governance
* Knowledge of National policies
* Knowledge of Audit and critical appraisal


Additional

* Speak English to an appropriate standard relevant to their role, i.e. with confidence and accuracy, using correct sentence structures and vocabulary, and without hesitation.
* Ability to work both autonomously and as a team member
