The Assistant Practitioner Therapy role within the Frailty team involves assessing patients and providing treatment and discharge plans. The role requires working across teams to support patients' hospital journey and facilitate safe discharge. It is a dual-trained assistant practitioner position working closely with physiotherapists, occupational therapists, and other assistant practitioners. The primary goal is to conduct short-term assessments and plan for discharge from hospital.

Main duties of the job

The successful candidate will work from 08:00 to 20:00 in a 7-day service, managing a caseload within a competency-based framework. Responsibilities include gathering baseline information for comprehensive assessments, working independently or with qualified therapists, and planning, reviewing, and modifying treatment in line with client goals. The role also involves managing risks, engaging in training and supervision, and contributing to audits and resource development.

Person specification

* Level 5 FdSc in an appropriate subject
* Additional qualifications in relevant clinical areas
* Significant multidisciplinary healthcare experience
* Experience in presenting to groups
* Caseload management experience
* Proficiency with System One
* Ability to travel within the geographical area, access to transport, and own vehicle
