An exciting new opportunity has arisen for a General Practitioner to join our team of forward-thinking, multi-disciplinary staff for the PCN, which includes Social Prescribing Link Workers, Care Coordinators, Clinical Pharmacists, Pharmacy Technicians, Cancer Care Facilitators, Advanced Nurse Practitioners, and Primary Mental Health Nurses.

Those due to obtain their Certificate of Completion of Training (CCT) in the coming months are welcome to apply.

UNFORTUNATELY, WE ARE NOT IN A POSITION TO SUPPORT TIER 2 VISA APPLICATIONS.

The successful candidate will be responsible for working through their own rota and will undertake a variety of duties including surgery consultations, telephone and video consultations, queries, and visiting people at home if required. They will be responsible for following up their own investigations and will benefit from regular supervision and reflection sessions with GP colleagues.

We are committed to the ongoing development of our team, which includes mandatory and developmental training for all staff. Team members are encouraged to highlight any training they feel would benefit them in achieving their role.


Main duties of the job

The GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced Services including:

* Share of routine and urgent appointments both face-to-face and telephone.
* Prescribe as per local & national guidelines.
* Share of home visits if required.
* Manage completion of administration: letters, results, tasks, referrals, telephone calls, emails.
* Take responsibility for appropriate referrals (following local guidance and good medical practice) and related administration.
* Participate in Extended Hours between 6.00pm - 8.30pm if required by the PCN.
* Collaborate with other health and social care providers to ensure a safe and integrated service. Provide comprehensive assessment, advice, and treatment for the registered practice population.
* Maintain accurate and comprehensive records of consultations using SystmOne.
* Provide clinical support to other staff members, both clinical and administrative, to achieve high standards of care and meet QOF and Enhanced Service targets.
* The content of this post will be reviewed in consultation with the post-holder when necessary and in line with service developments.

Sedgefield North Primary Care Network is an innovative, dynamic PCN comprising 5 local GP Practices based in Spennymoor, Ferryhill & Chilton, Sedgefield, and West Cornforth, serving over 58,000 patients. Job responsibilities

CLINICAL

* Record clear and contemporaneous IT-based consultation notes to agreed standards.
* Provide safe, evidence-based, cost-effective, individualized patient care within the hub, surgery, or patients' homes.
* Clinically examine and assess patient needs from physiological and psychological perspectives, and plan clinical care accordingly, referring patients to other services/agencies in a timely manner following local guidelines.
* Recognize the need for and perform and interpret investigatory procedures, including laboratory and radiology.
* Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations, including initiating effective emergency care.
* Ensure appropriate follow-up of patients.
* Support patients in adopting health promotion strategies that promote healthy lifestyles and apply principles of self-care.
* Support and collaborate with colleagues to deliver anticipatory care plans.
* Work within your competence and in accordance with professional codes of conduct, maintaining accurate and up-to-date health records.
* Independently prescribe for patients within your scope of practice, following locally or nationally agreed guidelines.
* Review medications following appropriate policies, NICE/local clinical guidelines, and care pathways.
* Maintain awareness of developments in clinical practice.
* Ensure compliance with all relevant practice/PCN policies and guidelines, including prescribing, confidentiality, data protection, health and safety, and QOF standards.
* Be aware of your duties and responsibilities regarding current legislation and adhere to policies and procedures on safeguarding children and adults.

You will have access to confidential information relating to patients, carers, practice staff, and other healthcare workers. Patients entrust us with sensitive information about their health and other matters, expecting us to respect their privacy and act appropriately. Information may only be divulged to authorized persons in accordance with practice policies and procedures relating to confidentiality and data protection.

HEALTH AND SAFETY

You will promote and maintain your own and others' health, safety, and security as defined in the practice's Health & Safety Policy, including:

* Using personal security systems within the workplace according to PCN guidelines.
* Identifying risks involved in work activities and managing those risks.
* Utilizing training to update knowledge and skills.
* Following infection control procedures, including maintaining a tidy and hazard-free work area and practicing effective hand hygiene.
* Providing advice on the safe management of specimens, including collection, labeling, handling, storage, and transport.
* Using personal protective equipment (PPE) correctly in routine and extraordinary circumstances.
* Managing incidents of accidental exposure.
* Safe use, storage, and disposal of sharps.

Person Specification


Qualifications

* Newly qualified GP (within 2 years and not having held a substantive post) with full GMC registration and license to practice.


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a Disclosure check for any criminal convictions.

Depending on experience, £10,000 per session.
