Specialist Physiotherapist - Adult Haemoglobinopathies

The post holder will be part of the NCL Red Cell Community Team supporting the implementation of evidence-based care pathways and rehabilitation services for patients with Sickle Cell, Thalassaemia and Rare Inherited Anaemias in NCL and help further develop the Red Cell Community service. The post holder will provide first-class and evidence-based patient care and promote health through patient empowerment, health education and self-management in the hospital, patients' homes and at Camden & Islington Sickle Cell & Thalassaemia Centre.


Main duties of the job

* Provide support as a Red Cell Community key worker to a caseload of patients through regular home visits and telephone contact to improve quality of care and to support admission avoidance.
* Provide specialised physiotherapy to patients who are case managed by other members of the team.
* Work in close collaboration with the Whittington Health Haematology consultants, the Community Team, GPs and community services to provide a comprehensive and seamless programme of care for suitable patients.
* Attend regular multidisciplinary team meetings including the hospital-based team meeting to review patient management, new referrals, and discharges; and, team meetings to share learning, reflective practice, and education.
* Work as a practitioner assessing patients in their own homes, health centres and other community settings. The post holder will work flexibly and in close liaison with the hospital Haematology team to ensure continuity of service delivery.


Person Specification


Education / Qualifications

* o Registered Therapist (with the HCPC)
* o Understanding of integrated care
* o Masters level education or equivalent experience
* o Specialist qualification or course in haemoglobinopathies
* o Leadership or mentorship course


Skills & Abilities

* o Able to manage competing demands and priorities in a changing, complex environment and is personally resilient
* o Able to critically analyse services and systems
* o Competent and skilled in moving and handling techniques and familiar with a wide range of equipment to support this e.g. hoists, sara steady, slide sheets etc
* o Competent IT skills including Microsoft Office apps (Word, Outlook, Excel, PowerPoint)


Knowledge & Experience

* o Wide range of B5 and B6 rotations or equivalent senior, experience in specialism or related area
* o Experience in pain management
* o Evidence of management of change, clinical leadership, and development of services
* o Experience of working in the community
* o Experience participating in audits and clinical research
* o Experience of multiagency working
* o Minimum 4 years post-graduate experience with minimum 2 year at band 6 level in an NHS in-patient setting
* o Experience managing patients with red cell disorders in the community
* o Experience of acute sickle cell crisis management

* o Management skills including experience of team leadership and team building initiatives
* o Able to manage staff recruitment, appraisal, and performance management


Communication

* o Excellent interdisciplinary communication skills and ability to facilitate collaborative multidisciplinary working relationships
* o Strong written and verbal communication skills, with ability to adapt to effectively manage complex and sensitive situations


Other

* o Ability to travel independently to all sites across NCL
* o A flexible approach to service delivery with ability to work evenings and weekends on occasions where required
